Clinical Opportunities (Adult and Pediatric ID):

Many complex infectious diseases cases are encountered at the Tikur Anbessa Hospital, the largest tertiary care and referral centre in Ethiopia. In addition to the high burden of HIV and opportunistic infections, particularly cryptococcosis and TB, visiting fellows will have exposure to a wide range of clinical cases including relapsing fever and leishmaniasis. A major challenge of this rotation is to learn how to manage complex ID cases with limited microbiological data and other laboratory resources.

Expected Length of Observership: Between 2-4 weeks, other durations negotiable.

Application Requirements:

Applicants should send a cover letter detailing their objectives and an updated CV. The complete application should be sent directly to the local program director and the external coordinator. Rotations should be planned with a minimum of 3 months advance. Visiting fellowships have to be approved by the Department of Medicine Addis Ababa University.

Costs/Fees, Housing and Logistics:

There is no university housing available for visiting fellows; however a number of hotels and guest houses are available within reasonable proximity to the hospital. Administrative or university fees are not imposed on visiting fellows, but transport, housing and all living expenses are the sole responsibility of the visiting fellow and expenses are not funded by the university or the hospital.

Clinical Opportunities:

80% of the work of the Infectious Diseases unit is HIV-related, with extremely high levels of HIV-TB coinfected patients. Full range of developing country HIV-OI comorbidities. General ID an increasing focus of the Unit, with hospital-wide antibiotic stewardship program, and outreach to several secondary level hospitals and district hospitals providing specialist tuberculosis services. Small renal transplant program including HIV+/+ transplants. GeoSentinel Travel Surveillance Network Site.

Patient Interactions with Observer:

Interview and examination. Not allowed to perform invasive procedures as an observer.

Local hostels in close proximity to the hospital. No official University of Cape Town accommodation available.

Cost/Fees Associated with Observership:

No University fees. Cost of observership is entirely the responsibility of the fellow. There is no funding available for transport, accommodation or daily living expenses.

Participants join the Infectious Diseases team in the daily clinical activities of the Department of Infectious Diseases and Tropical Medicine of Cayetano Heredia Hospital that includes activities with out-and in-patients.

An email stating the period of the year requested, a photograph, and a copy of passport are required. Acceptance will be entirely on a first come-basis. U.S. citzens do not require a visa.

Housing Availability:

We will provide a list of optionsl most of the options are family houses of former-UPCH medical students. Support will be provided in locating the best available housing options. For a family house the cost will be around $300 USD a month. UPCH offers housing at $600 USD a month. Daily cost for meals is approximately $10 USD a day and public transport is $2 USD a day in Lima.

Rotating fellows will work with the general ID inpatient service as well as be involved in the clinics. As the Aga Khan University Hospital (AKUH) is a large tertiary-care, multi-disciplinary, referral center, in addition to seeing complex general ID infections, fellows should expect to see a variety of endemic infections such as malaria, dengue, and enteric fever as well as infections due to resistant (predominant gram negative) pathogens. Moreover, ID fellows will be involved in the care of a large variety of extrapulonary TB, which form the bulk of the patients presenting to the ID clinics. However if more exposure to TB management is required, arrangements can be made to also rotate at a large MDR-TB treatment center which provided DOTS-plus therapy. Finally while rotating at the AKUH, fellows will be expected to be involved in the hospital epidemiology, infections control and antibiotic stewardship activities.

Length of Observership: Minimum of 2 weeks with up to 6 weeks

Description of Patient Interactions:

Observers can interview and examine patients and will also have unrestricted accesses to the records of patients they are evaluating. However, observers cannot perform any procedures.

Visa to Pakistan can take up to 4 weeks. Given the high occupancy of the university managed hostels, it is advisable to request this 3 months in advance.

On campus (in the male or female dormitory) and off campus university managed hostels are available, though space is often limited. In case of off campus accommodation, a pick-up and drop-off service is provided. The estimated cost for accommodation is US $130 per month and includes transportation (if off campus) but not meals.

Application Requirements:

A general permission from the hospital will be obtained, after that there are no time constraints for the start of the observership, apart from visa etc.

We have a 1 week intensive course in ID in September (IDCC). This includes ward rounds, microbiology rounds, invited lectures, discussion on archived cases, MCQ sessions, panel discussions, case presentation by attendees etc. The observership may be best at a time which includes this course.

Housing Availability:

Housing can be arranged at a nearby hotel but will need to be paid for by the individual observer. The approximate tariff may be US $100 to $150.

Cost/Fees Associated with Observership:

The fees for IDCC are approximately US $150-$200. There are no other costs involved in the fellowship.
